About the Role
An exciting opportunity has arisen for a Lead Nurse to join our busy and dynamic South and Dales Older Adults Community Mental Health Team (OA CMHT).
We are seeking an experienced and compassionate clinician with a strong interest in older adult mental health care. The team provides assessment, treatment, and ongoing support to older adults experiencing functional and/or organic mental health conditions, as well as assistance for their carers.
This role will primarily focus on our Care Home Project, where you will:
Deliver independent “light touch” clinics within care homes.
Provide non-pharmacological advice, guidance, and support to care home staff.
Manage a complex caseload within the care home setting.
Conduct assessments and interventions in collaboration with the wider clinical team.
You will also participate in the duty rota, offering Single Point of Access (SPA) cover as required.
Key Responsibilities
Carry and manage a complex caseload of older adult patients.
Conduct thorough clinical and safety assessments, formulating person-centred care plans.
Undertake the role of duty worker, triaging referrals and coordinating with other services.
Participate in the Care Homes Project and Community Support Team (CST) meetings.
Supervise and support junior staff, including student nurses and healthcare assistants.
Contribute to multi-disciplinary discussions, ensuring holistic and recovery-focused care.
Maintain accurate documentation in line with Trust and NMC standards.
Promote best practice, wellbeing, and recovery-oriented care for older adults.
Professional Development and Leadership
This role offers excellent opportunities to develop leadership skills and to act as a link worker or clinical champion — such as a student placement link, medicines champion, or clozapine lead.
You will receive regular clinical and management supervision and be supported to grow both personally and professionally within the Trust.

Registered Nurses, Nurse Prescribers (NMPs), Occupational Therapists, Psychologists, and Psychology Assistants
Consultant Psychiatrists and Junior Doctors
Administrative and support staff
