JOB SUMMARY

 

The Hospital Discharge Team has recently changed its operations and administration. This post will provide experience working across health and social care, collaborating closely with health colleagues. An excellent opportunity for an experienced social worker to demonstrate and improve their leadership and management abilities.

As a Team Manager, you will be responsible for leading and directing our professional Social Workers and Social Care Assessors. You will be responsible for managing a team of Practitioners who work on a variety of caseloads such as care and support needs assessments, attending board rounds and collaborating with health colleagues on discharge plans, mental capacity assessments, risk management, safeguarding enquiries, and potential court work.

You will also coordinate with community team managers and health and social care personnel to ensure that STS evaluations and reviews are completed on schedule. You will supervise and support practitioners in promoting independence and implementing an asset-based approach that aims to maximise chances for people to decrease and delay their need for formal care and support. You will have experience protecting adults because the Team Manager will conduct safeguarding investigations in accordance with organisational policy and guidance.

You will play an important part in ensuring that our clients receive the help they require while also establishing a good and collaborative work environment.
