JOB SUMMARY

We are searching for an exceptional Social Worker to join our Adult Social Care Operational Teams at The Conquest Hospital.
We have great partnerships with local partners and hospital trusts. We work together to help people safely discharge from hospitals and return to their homes in the community. We advocate for the home first model and the discharge to assess strategy, believing that with the proper amount of care, clients may keep their independence and stay at home as safely as possible. We collaborate closely with our reablement teams and inpatient rehabilitation providers to maximise client independence while also instilling confidence and achieving excellent outcomes. Where appropriate, we collaborate with our local Integrated Commissioning Boards (ICBs) to ensure that the people we serve have access to continuing care funds.
To carry out the function, you must have a thorough understanding of the Mental Capacity Act (2005), the Mental Health Act (1983), Equality legislation, and Deprivation of Liberty provisions. You must be able to use this legislation in your daily practice. You will also be required to carry out the County’s section (42) safeguarding duties in order to protect and minimise risks to the clients we serve. You will be asked to assess clients in accordance with our other legislative requirements, such as section (9) assessment under the Care Act (2014), employing a strength-based, outcome-focused approach. Experience in court of protection work would be advantageous in this post, however training is provided to successful candidates.
You must be a Registered Social Worker with Social Work England and hold a social work qualification, such as a C.Q.S.W, DipSW, Degree, or Master’s in Social Work. Social workers with Assessed and Support Year Employment credentials are encouraged to apply.

If you are applying for a part-time or term-time position, your starting salary will be pro rata.
