Overview

Livewell Southwest is looking for a motivated and compassionate Registered Mental Health Nurse (or soon-to-qualify applicant) to join our Functional Older People’s Mental Health (OPMH) community team. This is an excellent opportunity to work with people aged 65+ living with functional mental health conditions such as depression, anxiety, bipolar disorder, and schizophrenia—supporting them to maintain independence and wellbeing within the community.

What You’ll Do

In this varied community role, you will:

  • Conduct holistic mental health assessments in patients’ homes or clinic settings.
  • Develop personalised care plans in line with the Personalised Care Framework.
  • Provide therapeutic nursing interventions and monitor ongoing mental health needs.
  • Manage your own caseload and act as a keyworker for individuals under your care.
  • Respond to duty calls and coordinate with psychiatrists for medication queries.
  • Work closely with a range of partner services, including inpatient units, GPs, social care, voluntary organisations, and families to deliver joined-up support.
  • Maintain accurate, confidential clinical documentation in line with professional and legal standards.
  • Contribute to service audits, peer reviews, and quality improvement activities.
  • Offer supervision to junior colleagues and support the learning of students on placement.

Key Professional Requirements

Essential Qualifications

  • Current NMC registration as a Mental Health Nurse (or pending registration for newly qualified applicants).

Knowledge & Understanding

  • Mental Health Act and principles of confidentiality
  • Care Programme Approach / Care Coordination

Mental Capacity Act & DoLS

  • Awareness of NICE mental health guidelines
  • Understanding of major psychiatric conditions and evidence-based interventions

Experience

  • Evidence of multidisciplinary working
  • Experience supporting adults with severe and enduring mental illness
  • (Desirable) Experience within a community mental health setting

Skills

  • Ability to work autonomously while contributing to a wider team
  • Strong communication and liaison skills
  • Assessment, risk management, and crisis intervention abilities
  • Competent use of digital clinical systems
  • Ability to travel across Plymouth (reasonable adjustments considered)
