This position comes at a great moment, as we’re enhancing our services through a trust-wide refresh of the dementia pathway. Our goal is to make specialist memory assessments more accessible and quicker, helping people receive a timely diagnosis. Early diagnosis opens doors to better support, treatments, and planning, ultimately improving quality of life. What You’ll Do Day-to-Day

You’ll play a key part in delivering excellent community mental health care for older adults, working closely with individuals, their loved ones, and both paid and unpaid carers. Strong skills in autonomous working, teamwork across disciplines, and partnering with external organisations are vital.

Key responsibilities include:

  • Making first contact with service users to guide them through the memory pathway with empathy and clarity.
  • Conducting thorough, person-centred assessments, gathering detailed histories while actively involving families and carers to co-design care.
  • Accurately documenting assessments and discussions in our systems to support safe, informed decisions in multidisciplinary meetings.

In return, expect regular supervision (clinical, management, and caseload), appraisals, and mentorship from our experienced leadership, including the Team Manager and Clinical Specialist Nurse/Advanced Practitioner.

You’ll need independent travel across the area with access to a suitable vehicle (reasonable adjustments available under the Equality Act 2010).

Who We’re Looking For

Essential Qualifications:

  • Current NMC registration (RNMH or RNLD)
  • Commitment to ongoing professional development
  • Willingness to train as a practice assessor/mentor and clinical supervisor

Essential Experience:

  • Supporting people with mental health issues or learning disabilities
  • Person-centred approaches and collaboration with service users, families, and multi-agency teams

Essential Knowledge and Skills:

  • Best practices in the field, including relevant laws (e.g., Mental Health Act, Mental Capacity Act)
  • Safeguarding, risk assessment, and clinical governance
    Effective communication of sensitive information
  • Medicines management, including injections
