About the Role

As a Care Coordinator, you will ensure residents receive high-quality, person-centred care. Working closely with the care team, you will manage daily care delivery while supporting residents’ individual needs, preferences, and goals.

Key responsibilities include:

Leading and supervising the care team.

Coordinating shifts and delegating responsibilities.

Administering and recording medication.

Maintaining accurate, computerized care plans.

Liaising with healthcare professionals.

Supporting induction, training, and supervision of new staff.

Reports to: Home Manager

Skills & Experience

Previous experience supervising a care team.

Compassion, empathy, and strong understanding of resident needs.

Effective leadership, communication, and teamwork skills.

Organized, methodical, and detail-focused.

Flexible approach to schedules and responsibilities.

Qualifications

NVQ Level 3 in Health & Social Care (or working towards).
