What it’s like to work with our team…
You will be part of a joint team financed by both Health and Social Care, based at the Bracknell Community Mental Health Team’s offices. You will be responsible for assisting our carers and doing carer assessments.
Regular supervision, team, and MDT meetings are held to assist you, and your involvement is required.
You will be assisted in carrying out the following duties, responsibilities, and training…
● You will be supported in carrying out a caseload of carers evaluations as necessary by the service.
● We will assist you in adhering to the statutory framework necessary for a Community Mental Health Team, which includes the Mental Health Act 1983, Mental Capacity Act 2005, and the Care Act 2014.
● You will collaborate with carers and their families to create a person-centered assessment of carers’ needs and risks, and from there, you will create care plans that are centred on the carer.
● You will be encouraged to collaborate with all internal and external experts using efficient communication.
