This service focuses on supporting individuals with complex forensic mental health needs as they move from secure hospital environments back into community life. The role is both challenging and rewarding, requiring a balance of clinical expertise, risk management, and compassionate, recovery-focused care.
You’ll be part of a multidisciplinary team working across Somerset and the wider South West region, helping reduce unnecessary admissions while ensuring safe and structured transitions into the community.
What You’ll Be Doing
Caseload Management & Care Coordination
- Jointly manage a small but highly complex caseload (typically up to 15 individuals).
Support patients through their journey from secure care to independent community living.
Assessment & Risk Management
- Carry out detailed forensic mental health assessments.
Develop and maintain robust risk management and care plans.
Contribute to safe discharge planning and prevention of re-admission.
Multi-Agency Collaboration
- Work closely with professionals across health, social care, probation, prison services, housing, and justice systems.
Engage in multi-agency meetings, including MAPPA processes.
Patient-Centred Support
- Build therapeutic relationships with individuals who may be difficult to engage.
Support recovery through interventions addressing mental health, substance use, social needs, and daily living skills.
Professional Contribution
- Maintain accurate clinical records and documentation.
Participate in clinical audits, governance activities, and service development.
Support and mentor colleagues, students, and junior staff when needed.
Where You’ll Work
The role is based in Bridgwater, with travel across Somerset and the wider South West region, including visits to secure units. A flexible approach and ability to travel are essential.

Essential Requirements
Qualifications
- Registered Mental Health Nurse (RMN), Occupational Therapist, Social Worker, or equivalent health/social care qualification
Experience
- Solid experience in mental health settings (community and/or inpatient)
Experience working within multidisciplinary and multi-agency teams
Knowledge & Skills
- Understanding of forensic mental health practice
Awareness of relevant legislation, including the Mental Health Act and Mental Capacity Act
Strong communication, risk assessment, and care coordination skills
Desirable Attributes
- Experience managing complex forensic caseloads
Involvement in supervision or mentoring
Ongoing professional development in forensic mental health
Legal & Compliance Requirements
This role requires a Disclosure and Barring Service (DBS) check due to the nature of the work.
