Role Overview

This is a meaningful opportunity to join a community-focused healthcare team supporting people living with HIV across Sussex. As a Community HIV Support Practitioner, you will play a key role in delivering compassionate, person-centred care while helping to reduce stigma and improve access to services.

This is a varied role where no two days feel the same. You might be supporting someone to better manage their health, carrying out clinical tasks in the community, or contributing to awareness and education initiatives.

What You’ll Be Doing

Clinical & Patient Support

  • Carry out phlebotomy and other delegated clinical tasks
    Support individuals in managing long-term HIV care, including medication adherence
    Assist in delivering personalised care plans alongside specialist nurses
    Conduct community visits independently when appropriate
    Provide practical support, including accompanying patients to appointments

Communication, Education & Advocacy

  • Build trusting relationships with service users and carers
    Communicate sensitively around complex and sometimes challenging situations
    Deliver HIV awareness sessions within healthcare settings and the wider community
    Promote inclusion and actively challenge stigma associated with HIV

Administration, Audit & Quality

  • Maintain accurate and up-to-date electronic patient records
    Contribute to audits, research, and service improvement initiatives
    Support day-to-day administrative processes that keep the service running effectively

Key Skills & Attributes

  • Strong interpersonal and communication skills
    Confidence in clinical tasks (or willingness to develop skills such as phlebotomy)
    Ability to work independently and within a team
    Compassionate, non-judgmental approach to care
    Organised, proactive, and adaptable
    Good IT and record-keeping skills

Essential Requirements

Ability to travel across Sussex (full UK driving licence required)
