Are you a compassionate General Practitioner with a passion for palliative and end-of-life care? Minehead Medical Centre is looking for a GP to join our dedicated primary care team and provide specialist input for patients with advanced illness, supporting them in their homes, care homes, and the wider community.

What You’ll Do
As a GP with an interest in palliative care, you will:
- Provide generalist and specialist palliative care to patients in both community and practice settings.
- Lead anticipatory care planning, including advance care planning and DNACPR discussions.
- Support symptom management, medication optimization, and work to reduce avoidable hospital admissions.
- Act as a clinical lead for palliative care, supporting colleagues, contributing to service development, and enhancing care quality.
- Collaborate closely with multidisciplinary teams, including community nursing, specialist palliative care, social services, and secondary care providers.
- Engage patients and families with empathy and professionalism, ensuring a patient-centred approach in all care decisions.
- You will also have the opportunity to participate in portfolio work, delivering clinical services across multiple pathways and supporting continuous improvement within the practice.

What You’ll Need
Essential:
- Current GMC registration and inclusion on the NHS Performers List.
- Post-CCT experience in Primary or Urgent Care.
- Evidence of postgraduate training or experience in palliative care (e.g., Diploma in Palliative Medicine, RCGP End-of-Life Care modules).
- Proven experience in multidisciplinary collaboration and community-based care.
- Commitment to high-quality, patient-centred care and proactive management of clinical responsibilities.
Desirable:
- Leadership experience in primary care.
- Experience in portfolio-based roles or multi-modality clinical work.
