Minehead Medical Centre is looking for a General Practitioner with a special interest in palliative and end-of-life care to join our values-led team. This role blends high-quality general practice with a clinical leadership focus in palliative care — supporting patients and families at some of the most important moments of their lives.

Your Role in the Community
You will provide holistic, patient-centred care for people living with advanced progressive illness and those approaching the end of life, across both practice and community settings.
As the Palliative Care Clinical Lead, you will:
- Deliver generalist GP care alongside specialist palliative input
- Lead anticipatory and advance care planning, including DNACPR discussions
- Support symptom control, medication optimisation, and dignity-focused care
- Work closely with community nursing teams, hospices, specialist palliative care services, social care, and secondary care colleagues
- Help reduce avoidable hospital admissions through proactive, coordinated care
- Support colleagues through education, advice, and shared learning
- Contribute to service development, quality improvement, and PCN-level working
Who We’re Looking For
You’ll thrive in this role if you:
- Are a GMC-registered GP on the GP Specialty Register with inclusion on the NHS Performers List
- Have post-CCT experience in primary care or urgent care
- Can evidence postgraduate training or experience in palliative care
(e.g. Diploma in Palliative Medicine, RCGP End of Life Care modules) - Have experience supporting patients with complex, advanced illness
- Work confidently across multidisciplinary and cross-sector teams
- Are organised, proactive, and comfortable balancing clinical and leadership responsibilities
- Bring empathy, professionalism, and a genuine passion for person-centred care
