Overview
We are looking for a dedicated and experienced Mental Health Practitioner to join our Community Mental Health Service at Somerset NHS Foundation Trust. In this role, you will be part of a supportive multidisciplinary team delivering high‑quality, recovery‑centred care to adults with complex and enduring mental health needs.
You’ll be a trusted point of contact for individuals seeking help, providing timely support via phone and MS Teams, conducting assessments, and helping guide service users toward meaningful recovery outcomes.

Key Responsibilities
Clinical Support & Intervention
- Provide emotional support and crisis response through telephone and virtual channels.
- Undertake detailed assessments using recovery‑focused, strengths‑based approaches.
- Prioritise and triage referrals, gathering relevant information to guide clinical decision‑making.
- Develop, communicate, and update personalised Recovery Care Plans and Risk Management Plans.
Coordination & Communication
- Act as the initial contact for service users in distress — offering calm reassurance and practical, solution‑focused guidance.
- Work closely with carers, community partners, internal teams, and external agencies to coordinate care.
- Maintain accurate, timely clinical records in line with Trust and NHS standards.
Professional Engagement
- Participate actively in team meetings, supervision, reflective practice, and clinical governance.
- Contribute to service development through audits, quality improvement work, and shared learning.
- Model high professional standards, adapting confidently to unpredictable situations and behaviours.
Who We’re Looking For
Essential Qualifications
- Registered Mental Health Nurse (RMN) or equivalent professional registration (must hold current UK registration).
- Demonstrable post‑qualifying clinical development in acute or community settings.
Required Experience
- Substantial experience working in multidisciplinary community mental health care.
- Proven ability to manage risk and deliver assessments, planning and evaluation collaboratively.
- Understanding of mental health conditions and best‑practice support strategies.
Other Requirements
Enhanced Disclosure and Barring Service (DBS) check (required under the Rehabilitation of Offenders Act – Exceptions Order 1975).
