About the Role

We are delighted to offer an opportunity for a Public Health Practitioner (Health Visitor) to join our Children and Families Care Group, working within the 0–19 Integrated Health Visiting and School Nursing Service across Stoke-on-Trent.

This role sits within Universal Services and plays a vital part in delivering the Healthy Child Programme, supporting children, young people, and families from pregnancy through adolescence. You will work as part of the South West 0–19 Locality Team, contributing to early intervention, prevention, and population health improvement.

This is a varied and rewarding role for a practitioner who enjoys working across community settings, values partnership working, and is committed to reducing health inequalities while improving outcomes for children and families.

What You’ll Be Doing

As a Specialist Community Public Health Practitioner, you will work autonomously within your professional scope while being fully supported by Team Leaders and the Professional Lead. You will take responsibility for a defined population and contribute to high-quality, accessible services across localities.

Key aspects of the role include:

  • Delivering commissioned elements of the Healthy Child Programme (0–19)
  • Assessing the health, development, and wellbeing needs of children, young people, and families
  • Planning, implementing, and reviewing care and support packages across community and clinical settings
  • Working closely with multi-disciplinary and multi-agency partners, including education, social care, and voluntary services
  • Supporting early identification of need and preventing avoidable hospital admissions
  • Promoting safeguarding, inclusion, and health equity in all areas of practice
  • Providing supervision, guidance, and leadership to colleagues where appropriate
  • Contributing to service development, quality improvement, and local performance outcomes
  • Demonstrating creativity, initiative, and problem-solving in response to complex family needs

Essential Qualifications & Registration

You must have:

  • Current Nursing and Midwifery Council (NMC) registration
  • Specialist Community Public Health Nurse (SCPHN – Health Visitor) qualification
  • or be currently undertaking an SCPHN programme
  • Eligibility to work within UK professional and regulatory standards

  • This role is subject to an Enhanced Disclosure and Barring Service (DBS) check
  • Skilled Worker visa sponsorship is available for eligible applicants
  • Overseas applicants must meet UKVI criminal record certification requirements
