About the Role
An exciting opportunity has arisen for a Senior Community Mental Health Practitioner to join the Teign Core Adult Community Mental Health Team, based in modern offices in Newton Abbot, with excellent transport links.
This role sits within a well-established, forward-thinking multi-disciplinary team (MDT) delivering high-quality, recovery-focused mental health care to adults with complex needs in the community.
We welcome applications from Registered Mental Health Nurses, Learning Disability Nurses, Occupational Therapists, and Social Workers who are passionate about collaborative working, person-centred care, and continuous service improvement.
As a senior practitioner, you will play a key role across the full care pathway, from triage and assessment through to treatment, review, and discharge, while also supporting junior colleagues and contributing to service development.
What You’ll Be Doing
In this senior clinical role, you will hold a diverse and complex caseload, working autonomously while remaining an integral part of the MDT.
Your responsibilities will include:
- Completing comprehensive mental health assessments and risk formulations
- Developing and reviewing person-centred care plans using goal-based outcomes
- Delivering evidence-based therapeutic interventions, both one-to-one and in group settings
- Supporting individuals through recovery-focused, strengths-based approaches
- Undertaking proactive risk assessment and safety planning
- Working flexibly and assertively to engage people who may find services difficult to access
- Maintaining accurate, timely, and high-quality clinical records
- You will be actively mobile across the locality to ensure equitable access to care for service users who are unable to attend clinic appointments.
Leadership, Supervision & Development
As a senior member of the team, you will:
- Provide clinical supervision and support to junior and unqualified staff
- Mentor and support students during professional placements
- Contribute ideas for service improvement and innovation
- Support training and development initiatives within the team
- Engage in reflective practice, clinical supervision, and ongoing professional development
- You will be encouraged to develop your specialist interests and will be supported with relevant training to enhance your skills and career progression.
Integrated & Collaborative Working
You will work closely with a wide range of partner agencies, including primary care, social care, voluntary sector services, and acute providers, ensuring coordinated and seamless care for service users.
Physical health is a key component of community mental health care, and you will contribute to physical health screening, including basic monitoring for individuals prescribed antipsychotic medication.

Who We’re Looking For
Essential Qualifications
- Registered professional qualification:
- RMN / RNLD
- Social Work degree
- BSc/Diploma in Occupational Therapy
- Or equivalent recognised mental health qualification
- Current UK professional registration with no restrictions
- Evidence of ongoing continuing professional development (CPD)
- Ability to travel independently across the service area
Experience & Knowledge (Essential)
- Significant post-registration experience within mental health services (Band 5 or above)
- Experience managing a busy and complex community caseload
- Experience supervising staff and supporting student learning
- Strong understanding of the Mental Health Act, Mental Capacity Act, and national standards
- Experience working within MDTs and multi-agency settings
- Demonstrable commitment to recovery-oriented practice
Desirable
- Leadership or management training
- Knowledge of community and social care systems
- Postgraduate qualification relevant to your professional background
- Clinical supervision qualification
- Safeguarding Children Level 3 training
- Phlebotomy skills
