The Role
We are seeking an experienced and compassionate Senior Community Mental Health Practitioner to join our Community Mental Health Team. This is a key clinical role focused on coordinating care for individuals with complex, severe, and enduring mental health needs within the community.
Working under the guidance of the Deputy Team Manager or Team Manager, you will take professional responsibility for a complex caseload while also providing supervision and support to Band 5 Community Practitioners and Band 3 Community Healthcare Support Workers, where appropriate.
This role is centred on recovery-oriented practice, collaboration, and shared decision-making, ensuring service users and their carers are actively involved in planning and reviewing care.
Key Responsibilities
In this role, you will:
- Coordinate and manage a complex caseload of service users with severe and enduring mental illness.
- Undertake comprehensive mental health assessments when clinically indicated.
- Develop, implement, and regularly review personalised care and risk management plans.
- Deliver recovery-focused therapeutic interventions in line with agreed care plans.
- Assess and manage risk, adapting plans responsively as needs change.
- Work collaboratively with service users as partners in their own care, promoting choice, autonomy, and personalisation.
- Use recognised recovery tools such as Wellness Recovery Action Plans (WRAP) and Recovery Star to support empowerment.
- Ensure service users access a full range of interventions, including physical health monitoring, whether delivered directly or via other services.
- Engage proactively with carers and families, recognising their role in recovery.
- Monitor changes in mental health needs and respond promptly to prevent deterioration.
- Participate fully in caseload supervision, team discussions, and service development activity.
About You
You will be a confident and experienced mental health professional with strong clinical judgement and a commitment to recovery-focused care.
Essential requirements:
- Current UK professional registration as a Registered Mental Health Nurse or equivalent Mental Health Practitioner.
- A recognised mentorship qualification.
Desirable experience:
- Previous experience working within a community mental health setting.
- Leadership training or experience supporting and supervising others.
You will be able to work calmly with complex presentations, demonstrate strong communication skills, and model compassionate, accountable practice aligned with Trust values.
