Overview
We invite applications for a Band 5 Mental Health Practitioner to join the Assertive Outreach team within the Child & Adolescent Mental Health Service (CAMHS) for the regions of Calderdale and Kirklees. This is a community‑based role focusing on supporting children, young people and their families who require intensive outreach support.
You will join a friendly, experienced and supportive multi‑disciplinary team, working flexibly to help young clients engage in life‑skills, education, social integration, and recovery‑focused therapeutic activities. Key Responsibilities
Work as part of an integrated multi‑disciplinary CAMHS team to assess, plan, implement, and evaluate therapeutic interventions for children and young people.
- Carry out comprehensive risk assessments and contribute to care planning, ensuring safeguarding and overall welfare of clients.
- Engage therapeutically with young clients — individually or in groups — to promote life skills, social inclusion, education and recovery.
- Collaborate with carers, families and other agencies to ensure holistic, coordinated care.
- Maintain high-quality case records (written and electronic), manage referrals, and contribute to evaluation and audit processes.
- Participate in regular supervision, peer support and, where appropriate, supervision of junior staff or trainees.
- Uphold statutory and professional standards, including adherence to relevant legislation and guidance such as the Mental Health Act 1983 and the Care Act 2014.
- Be flexible in working hours (standard hours: Monday to Friday, 09:00–17:00), including occasional extended hours if required.
Candidate Profile & Essential Criteria
Required
- Qualification as a Registered Mental Health Nurse, Social Worker or Occupational Therapist.
- Demonstrable experience in working with children/young people experiencing mental health difficulties.
- Competence in risk assessment, care planning and delivery of therapeutic interventions in a community setting.
- Comfortable working independently and within a team; strong communication and interpersonal skills; good IT literacy.
- A valid driving licence (outreach travel required).
- Commitment to safeguarding children and vulnerable individuals; ability to work under pressure and manage a defined caseload.
Desirable
- Previous mentoring or supervising experience; willingness to work toward practice‑educator status.
- Broad understanding of social, physical and environmental factors affecting mental health — and experience of recovery‑oriented, holistic support.
