Pacific Gardens Nursing and Rehabilitation is a trusted post-acute and long-term care facility in Fresno, CA, offering short-term rehabilitation, geriatric outpatient therapy, and skilled nursing services. We’re expanding our in-house therapy team and seeking a dedicated Speech-Language Pathologist (SLP) who shares our passion for empowering patients through exceptional, evidence-based care.
As a valued member of our interdisciplinary therapy team, you’ll enjoy a supportive environment that promotes continuous professional growth — including specialized training opportunities in dysphagia management (Think Thin), dementia and Abilities Care, neuro and stroke recovery, and respiratory rehabilitation.
Your Role & Responsibilities
- Evaluate and Diagnose: Conduct assessments for patients presenting with communication, swallowing, or cognitive impairments. Develop personalized treatment plans in alignment with clinical and regulatory standards.
- Deliver Therapeutic Interventions: Provide speech, language, and swallowing therapy, utilizing best practices and current evidence-based methodologies.
- Monitor & Document Progress: Assess treatment outcomes, adjust care plans as needed, and prepare accurate documentation including discharge summaries per state and federal guidelines.
- Patient & Family Education: Offer training and support to families and caregivers to promote long-term progress and carryover of therapy goals.
- Collaborative Care: Work closely with interdisciplinary staff to ensure holistic, patient-centered care and consistent quality outcomes.
Qualifications & Licensure
- Active Speech-Language Pathology license in the State of California (Required).
- Open to both new graduates and eperienced clinicians.
- Strong communication, empathy, and documentation skills.
- Familiarity with rehabilitation in skilled nursing or post-acutex care settings is an advantage.
