Job Description

Overview

The Managing Memory Service Team provides specialist care and support for people with memory problems and dementia across Gloucestershire.

This role is central to coordinating ongoing assessment, treatment, and support for people living with dementia, as well as their carers and relatives, who are referred to the service. You will work collaboratively with the memory services team and external partners to ensure individuals and their families receive the best possible care.

You will also take a lead role in conducting assessments, providing treatment, and delivering post-diagnostic discussions to service users and their carers.

Key Responsibilities

Carry out assessments and provide treatment for people with memory problems/dementia, along with their carers and families.

Lead on post-diagnostic discussions, ensuring service users and carers are supported and fully informed.

Work collaboratively with colleagues within the memory services team, maintaining strong professional links across the service.

Provide clinical supervision, caseload management, mentorship, and day-to-day oversight for designated staff groups and students, supporting their professional development and the delivery of high-quality care.

Offer clinical leadership and act as a role model, upholding high professional standards.

Ensure practice within the team is evidence-based, supporting continuous improvement and service development.

Engage with partner agencies, providing education, advice, and consultancy to help inform decision-making for service users and carers.

Lead education initiatives for users and carers, based on service needs.

Develop and implement local and national policies and procedures to uphold consistent quality standards and ensure the health and safety of patients and staff.

Work in partnership with statutory and non-statutory organizations to provide joined-up care.
