About the Role
We are looking for a caring, motivated, and enthusiastic Staff Nurse (Band 5) to join our Unplanned Care Team. This is a dynamic community-based role that focuses on providing rapid assessment and nursing support in patients’ homes. The aim is to prevent unnecessary hospital admissions and support safe, timely discharges from hospital.
You will work within a multidisciplinary team, managing your own caseload and collaborating with Community Partnerships and Primary Care Networks to deliver high-quality care.
The service operates 24/7 on a rotational basis with the support of agile technology, enabling timely and effective patient care.
Key Responsibilities
Provide high-quality nursing care to patients in their own homes.
Rapidly assess and manage presenting health needs to reduce hospital admissions.
Support and facilitate early discharges from hospital.
Work flexibly as part of a seven-day service covering 24-hour shifts.
Collaborate with a range of health and social care professionals to achieve the best patient outcomes.
Demonstrate effective clinical and communication skills to ensure a positive patient experience.
Actively contribute to service development and quality improvement.
About You
We are seeking a nurse who is:
NMC registered (applications also welcome from final-year student nurses).
Passionate about patient-centered care in the community setting.
A strong communicator with good clinical skills and enthusiasm for multi-agency working.
Able to manage their own workload while working effectively as part of a team.
Experienced in community nursing (desirable, but not essential as training and support will be provided).
Newly qualified nurses will be supported through a tailored preceptorship programme to aid professional development.
