About the Role
We are seeking an experienced Band 7 Registered Nurse with specialist expertise in diabetes care to join our established multidisciplinary community team. The successful candidate will play a key role in supporting service development while managing a caseload of patients with complex diabetes needs.
You will work across community and secondary care services, collaborating closely with diabetes consultants, dietitians, and other healthcare professionals to deliver high-quality, person-centered care.
Key Responsibilities
Deliver and support a Community Diabetes Service in line with the National Service Framework for Diabetes.
Manage a caseload of patients with complex diabetes presentations requiring specialist assessment, treatment, and ongoing management.
Provide structured patient education programmes to promote self-management and improve health outcomes.
Offer clinical support and advice to health and social care colleagues.
Contribute to service development and quality improvement within the diabetes service.
