About the Service

The Leeds and York Partnership NHS Foundation Trust (LYPFT) provides specialist health services for adults (18+) with learning disabilities across community, respite, and intensive support settings. We work closely with service users, carers, social care, voluntary organizations, and private providers to deliver care for individuals with complex needs, including:

Behaviors that challenge

Sensory and communication difficulties

Mental health needs and dual diagnoses

Multiple physical health conditions

The Role

We are looking for a dynamic and compassionate Band 6 Learning Disability Nurse to join our Assessment and Referral Team (ART) within the Community Learning Disability Teams (CLDT).

Your primary responsibility will be to triage referrals to the CLDT, assessing eligibility for specialist learning disability health services. This includes:

Gathering information from service users, careers, and professionals

Presenting cases to the MDT to determine referral outcomes

Making recommendations where referrals fall outside learning disability services

Conducting direct assessments and home visits when required

You will also contribute to the development of resources, support junior colleagues through supervision, and play an active role in service improvement projects and governance.

About You

We are seeking a nurse who is:

Registered with the NMC as a Learning Disability Nurse

Experienced in working with adults with learning disabilities and complex needs

Able to carry out person-centered, holistic assessments

Confident working autonomously and within multidisciplinary teams

Committed to promoting inclusion, equality, and high-quality care

Experience of working with diverse populations and advocating for social inclusion is highly valued.

Important Information

This post is based at St Mary’s Hospital and line-managed within the West and South CLDT.

Strong supervision structures are in place, including 1:1 clinical and managerial supervision, MDT peer support, and regular community nurse meetings.
