About the Role

We are looking for Neighborhood Clinical Assistants to join our Integrated Neighborhood Teams in West Leeds. This is a varied, rewarding role where no two days are the same, working directly with patients in their homes and community settings. You will play an important part in helping patients leave hospital earlier, preventing unnecessary admissions, and ensuring people receive high-quality care close to home.

You will be part of a multidisciplinary team, working alongside nurses, therapists, and self-management facilitators. Day-to-day duties may include:

Administering medication

Wound care and catheter care

Monitoring patient observations

Supporting patients with changing healthcare needs

You will be expected to work within your role’s scope, seeking support from the wider team when needed.

About You

We are seeking candidates who are:

Skilled communicators, with excellent organizational and IT skills

Compassionate, reliable, and able to respond to patients’ changing needs

Holders of a full UK driving license and access to a vehicle (essential)

Previous healthcare experience is beneficial but not essential – full training and a personal development plan will be provided. Career progression and apprenticeship opportunities are available.

Working Hours

The service operates 7am–10pm daily with a range of flexible shifts available. We understand the need for work-life balance and offer flexibility where possible.
