Purpose of the Role
To provide high-quality, person-centered care and support to individuals within a care home setting, promoting independence, dignity, and overall wellbeing. This includes supporting individuals with autism and other disabilities, ensuring their unique needs are recognized and met.

Main Responsibilities
Person-Centered Support
-
Promote the wellbeing, dignity, and quality of life of each resident.

Support individuals in daily routines and activities, both within the home and in the community.
-
Understand the specific needs of residents with autism and other disabilities.
-
Provide consistent and structured support in line with each individual’s care plan.
Key Working
-
Act as a key worker or co-key worker for assigned residents.
-
Coordinate care to meet the specific needs of your key resident(s).
-
Develop positive relationships with residents, their families, and advocates.
Daily Care Duties
-
Assist residents with personal care, ensuring privacy, dignity, and choice.
-
Support residents with daily living skills such as laundry, shopping, and meal preparation.
-
Maintain clear communication using appropriate methods for each individual.
Health & Wellbeing
-
Arrange and attend healthcare appointments with residents.
-
Administer and record medication according to company policy and training.
-
Promote healthy lifestyles and ensure physical and emotional needs are met.
Record Keeping
-
Accurately complete all necessary documentation, including:
-
Daily records
-
Care plans
-
Medication logs
-
Financial records
-
Communication logs (e.g., message books, diaries)
-

Maintain up-to-date inventories of personal belongings.
Teamwork & Communication
-
Attend and contribute to staff meetings, supervision, and appraisals.
-
Participate in regular reviews and planning meetings for residents.
-
Answer telephones professionally and relay messages accurately.
Health & Safety
-
Follow all Health & Safety policies, including fire safety and risk assessments.
-
Participate in fire drills and emergency procedures.
-
Take part in first aid training and duties as required.
Training & Development
-
Attend all required training sessions.
-
Keep personal training files and knowledge up to date.
-
Actively seek to understand autism and other conditions relevant to the role.
General Duties
-
Undertake waking night or sleep-in shifts as required.
-
Carry out shift leader responsibilities when assigned.
-
Ensure the home maintains a calm, structured, and supportive environment.

Adhere to all organizational policies and procedures.

Additional Information
This role involves working closely with vulnerable individuals, and therefore a DBS (Disclosure and Barring Service) check is essential. Full training will be provided.
