JOB SUMMARY

 

University Hospitals Birmingham NHS Foundation Trust is one of the country’s major acute hospital trusts, serving a varied population of 1.2 million in Birmingham, Solihull, and adjacent areas.

The Heartlands Hospital’s Older Persons Assessment and Liaison Team is looking for a highly enthusiastic and driven clinical nurse specialist who is passionate about caring for older adults.

As part of a multidisciplinary team, OPAL conducts prompt patient-centered assessments to people with urgent needs at the Heartlands Hospital. We offer effective and efficient multidisciplinary complete geriatric assessments in the ED, OPAD, and MAU.

Our goal is to prevent needless admissions by proactively assessing patients and resolving concerns such as function, mobility, and social requirements, directing them to community services as needed.

The role is based in the OPAD, MAU, and ED, and work will be done in these areas. The service is available 12 hours per day, seven days a week.

The team takes a home-first strategy, conducting client-centered assessments. You will use your leadership, experience, and counsel to assist patients throughout their journey.

Main responsibilities, tasks, and abilities required

As part of a multidisciplinary team (MDT), OPAL conducts prompt patient-centered assessments to people with urgent needs at Queen Elizabeth Hospital. You will conduct effective and efficient multidisciplinary comprehensive geriatric assessments (CGAs) in the Emergency Department and Acute Medical Unit.

We anticipate applicants to have strong communication skills, a flexible attitude, and the capacity to work independently. Excellent organisational and time-management abilities are necessary.

You will be able to prioritise tasks effectively and practise client-centered care. You should give documentation of applicable courses and training, as well as a commitment to continuous professional development.

You should have relevant clinical expertise for the position, as well as experience working in the acute NHS and in a multidisciplinary team.

The selected individual will be expected to supervise staff as well as assist undergraduate students with their clinical placements.

To satisfy the service needs, you will be needed to work long days (0730 – 2000), in accordance with your contracted hours.

Qualifications

Essential

  • Registered Adult Nurse on the NMC Register.
  • 18 month band 5. Experience

Desirable

  • Qualified in healthcare management and leadership
  • Health Assessment Course
  • Experience working in an acute setting.

Experience

Essential

  • Specialist knowledge of frailty disorders and therapies for older persons
  • Experience working at band 5 level or above in the NHS.
  • Excellent understanding of working in an acute trust.
  • Experience in supervising and managing subordinate staff
  • Can demonstrate understanding, expertise, and willingness to be a practice-based supervisor or assessor.
  • Demonstrate skill, experience, and passion for the following: leadership, team building, time management, role modelling, change management, clinical governance, audit and research, NHS policy and political awareness.
