About the Role
East Cheshire NHS Trust is seeking motivated and dynamic individuals to join our Patient Flow Team as Discharge Coordinators. Based at Mackle shield Hospital, you will play a key role in ensuring safe, timely, and effective discharges across hospital wards.
Working closely with the multidisciplinary team (MDT), you’ll coordinate care and help prevent delays in patient discharge, supporting seamless transitions and improving overall patient experience.
Key Responsibilities

Actively monitor and manage discharge plans for patients across designated wards.

Attend board rounds and act on information shared by the MDT to support timely discharge planning.

Facilitate early discharges, including transfers to the discharge lounge when appropriate.

Liaise with patients and families to manage expectations and address discharge-related concerns early.

Coordinate referrals and assessments, escalating any outstanding or delayed actions.

Ensure all discharge documentation is completed accurately and in a timely manner.

Support the discharge doctor with identifying patients for review and finalizing discharge summaries.

Work with bed/site management teams to provide accurate, up-to-date discharge forecasts.

Organize patient transfers to the discharge lounge and arrange transport when needed.

Collaborate with medical outlier teams to ensure out-of-specialty patients receive timely reviews.

Help maximize bed availability by ensuring beds are prepared for new admissions promptly.
About You

Strong interpersonal and communication skills.

Able to work collaboratively with clinical teams and patients’ families.

Organized, proactive, and confident in coordinating care activities.

Comfortable managing multiple priorities in a fast-paced environment.

Previous experience in healthcare, discharge planning, or patient coordination is desirable.

This role requires an Enhanced DBS with Barred List check. Overseas police clearance is also required if you’ve lived or worked abroad for 6+ months in the last 5 years.
