Job Summary – Community Mental Health Practitioner (RMN or Occupational Therapist)
About the Role
We are seeking Registered Mental Health Nurses (RMNs) or Registered Occupational Therapists to join our new integrated model of care – Mental Health Together. This is part of Kent and Medway’s Community Mental Health Transformation, aimed at providing person-centred, joined-up care for people with complex mental health needs.
You will work within a multi-disciplinary team (MDT) across health, social care, voluntary and community services to deliver evidence-based mental health interventions in community settings. Your role will focus on recovery, relapse prevention, and empowering patients to manage their mental health within their own environments.
Key Responsibilities
Clinical Care
Conduct holistic assessments and deliver personalized, trauma-informed interventions
Develop and manage individual care and risk plans in collaboration with service users and carers
Provide timely, needs-led support to reduce hospital admissions and support safe discharge
Ensure physical health checks and follow-up care are completed
Work flexibly across primary care, mental health teams, and voluntary sectors
Maintain accurate and timely records using Rios or similar clinical systems
Teamwork & Collaboration
Work closely with inpatient units, crisis services, GPs, and voluntary organizations
Participate in MDT discussions and care planning
Support early discharges and crisis intervention when needed
Leadership & Supervision
Supervise and mentor Band 5 staff and students
Act as a positive role model, encouraging high standards of care and professionalism
Participate in staff performance reviews and contribute to service development
Communication
Communicate clearly with service users, careers, and colleagues across services
Handle sensitive and complex information appropriately and professionally
Engage service users and careers in decision-making and care planning
Education & Training
Act as a mentor to students and new staff
Engage in regular supervision, appraisal, and CPD
Contribute to staff training, development of care protocols, and service improvement initiatives
Person Specification
Essential
Registered Mental Health Nurse (RMN) or Registered Occupational Therapist (HCPC)
Minimum of 2 years post-registration experience, ideally in a community setting
Experience delivering evidence-based mental health interventions
MELPS or equivalent mentoring qualification
Ability to work autonomously and within a team
Excellent communication and interpersonal skills
Desirable
Experience working in integrated care or transformation projects
Familiarity with RiO clinical system or similar
Understanding of trauma-informed care and strength-based models
