To be considered for this position on the team, you must have extensive clinical community experience as well as outstanding knowledge of community pathways. You will be responsible for a diverse workload of fast interventions, and you will get referrals directly from ED, AMU, and other front-door Acute services, initially under the supervision of the Clinical Service Lead. You will collaborate with Adult Services to establish and support this client group’s ability to remain at home with expanded services or access to Clinical Community services in order to avoid hospital admission or expedite hospital discharge. You must have previous experience working in community services at the Band 6 level, be able to work flexibly to suit the needs of the service, and collaborate well with partnership agencies at Social Services, Acute Trust, and third-party organisations.

Walsall Healthcare NHS Trust is an integrated trust and the sole provider of NHS acute care in Walsall, servicing a population of 286,700. It provides inpatient and outpatient treatments at the Manor Hospital, as well as a variety of community services. Walsall Manor provides the entire range of district general hospital services under one roof. The £170 million expansion was completed in 2010, and ongoing upgrades guarantee that the Trust now has cutting-edge Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit capabilities. Collaborate with GPs, experts in secondary care, and advanced nurses.

Practitioners will make diagnoses, create care plans, assess very complicated components and scenarios, and initiate follow-up for persons with acute diseases.

Negotiate and agree with patients, carers, and other care providers on individual roles and responsibilities in terms of actions to be taken and outcomes to be attained.
