JOB SUMMARY

 

We are seeking a registered nurse to join our established ward team. Someone who is passionate about giving excellent care to our patients.

Sheldon unit is a 23-bed medical unit focusing in rehabilitation and elderly care.

We are dementia friendly on Sheldon, and our charming poppy lounge allows residents to relax and recollect. We have an activity coordinator and a discharge coordinator on our staff.

Our goal is to provide world-class patient care, and we need skilled, motivated, and enthusiastic individuals to help us achieve it.

Responsible for delivering patient-centered care. Maintaining professional knowledge and clinical abilities in the fields of medicine and rheumatology while providing direct patient care. The position holder must be able to manage periods of moderate physical exertion during the shift worked.

Assist the ward manager in providing clinical and professional guidance, expertise, support, practice development, and performance monitoring.

Keep up with current political issues in the NHS and future nursing developments.

To provide leadership to all employees while encouraging a culture of positive and successful teamwork.

Assess care needs, requesting medical assistance as needed, and developing appropriate care plans for the patient, executing and assessing care during admission and in preparation for safe discharge. Supported by written evidence.

To deliver high-quality nursing care based on a thorough assessment of the patient’s needs, in collaboration with their families and the multidisciplinary team.

To continuously review the quality of care provided and re-assess the patient’s needs on a regular basis, in cooperation with the patient, other carers, and the multidisciplinary team, in order to effect the necessary changes to meet planned goals.

Collaborate with other professionals to provide comprehensive patient care.

Prepare well-planned discharges with adequate follow-up, while maintaining efficient communication with the family, Trust Allied Health Professionals, and community resources.

Maintain a patient/client-centered approach to care.

Ensure that the patient’s family, carers, and staff have their customs, values, and spiritual beliefs respected. To uphold the Trust’s Diversity and Equal Opportunities Policy.

Ensure the protection of the patient’s things.

Providing and receiving complex or sensitive patient information, as well as dealing with confrontations in which you can reassure, calm, and diffuse tough circumstances.

Provide advice on health promotion problems that are within the scope of your own profession.

Identify the patient’s, relatives’, and staff’s learning needs and, in cooperation with the Ward Manager, design and administer education to satisfy those needs.

Facilitates and maintains patient records to ensure that information is correct and up to date for future reference, in accordance with the Data Protection Act and NMC Patient Record Keeping Guidelines, and produces care plans to ensure that patients receive appropriate care.

To be able to properly manage time, ensuring that patients are prepared for planned treatments and that employees take adequate breaks during the workday and finish on time.

Ensure the safe delivery of medicines via the prescribed route in accordance with national and Trust requirements.

Assist the Ward Manager in ensuring that the ward environment fulfils the required standards, collaborating with all health care specialists.

The ability to utilise and recharge intravenous drug administration pumps, including recognising early indicators of a drug reaction and taking appropriate measures.

Frequent provision of patient care necessitates the utilisation of physical effort to roll, manoeuvre, and stand patients. Use manual handling equipment and training on the ward to reduce physical strain and ensure patient safety. Annual manual handling training is required.

Frequent treatment of waste bodily fluids to ensure proper disposal.

Participate in a yearly review with the Ward Manager to establish personal goals for your professional growth plan.

Attend annual required training to ensure compliance with Trust policy and current legislation.

Attend relevant in-house training programmes that are related to own clinical practice, to develop knowledge and skill, maintain competences.
