Our Community First Team is looking for Community Health and Social Care Assistants to join us in Derby City.

What does the Community First Team do?

We come in to help persons who have been discharged from acute hospitals or who require an immediate community response. The individual requires short-term/interim support in their home. The teams are dedicated to caring for and assisting people at home and work on a seven-day rotation.

What are the hours?

Various shift patterns.

(This includes unsocial hours, weekends, and bank holidays)

Where would I work?

Base: Florence Nightingale Community Hospital.

Area covered: Derby City and adjacent areas.

Working on this team means you must be able to meet the travel requirements of visiting people’s homes. You must have a UK driver’s licence and access to a vehicle. DCHS offers a mileage policy and a car lease programme to assist our employees.

The selected candidate is expected to work and document to a high standard while reflecting DCHS values.

To maintain standards, provide quality patient care by developing a breadth of knowledge and competencies to assist clinicians.

To help all clinicians in implementing agreed-upon treatment and care programmes as needed, as well as to function effectively as a member of the larger multidisciplinary team.

To provide individual care in the community without direct monitoring.

To help patients improve their health and freedom.

Be willing to assist Perth House in Chaddesden as staffing needs arise.

What is the purpose and activities of DCHS?

As part of Joined Up Care Derbyshire, we aim to deliver community health services to over one million patients in Derbyshire and Derby.
