Role Overview
An exciting opportunity is available for an experienced Advanced Clinical Practitioner (ACP) to join a community mental health service supporting adults across the Derwentside, Stanley, and Chester-le-Street areas.
This role is ideal for a skilled clinician who wants to take on a senior clinical position while contributing to service innovation and patient-centred care. As an ACP, you will work with a high level of autonomy, assessing and managing individuals with complex mental health needs while collaborating closely with multidisciplinary teams.
The successful candidate will play an important role in improving patient outcomes, supporting colleagues, and contributing to the future development of community mental health services.
About the Service
The service is part of Tees, Esk and Wear Valleys NHS Foundation Trust, which provides mental health and learning disability services across County Durham, Darlington, Teesside, North Yorkshire, York, and Selby.

Key Responsibilities
Advanced Clinical Practice
- Conduct detailed clinical assessments for adults with complex mental health conditions.
- Develop personalised treatment plans based on evidence-based practice.
- Diagnose conditions and initiate appropriate interventions within your professional scope.
- Prescribe medications and treatments where authorised.
Leadership and Service Development
- Contribute to the improvement of clinical services and care pathways.
- Lead quality improvement initiatives within the team.
- Provide guidance and mentorship to junior clinicians and trainees.
Education and Professional Support
- Support the learning and development of staff and students.
- Participate in teaching, supervision, and clinical training activities.
- Research and Evidence-Based Practice
- Promote research-informed clinical practice.
- Participate in service evaluations and clinical audits aimed at improving patient care.
Collaborative Working
- Work closely with professionals across primary care, community services, and specialist mental health teams.
- Build effective partnerships with patients, families, and carers to ensure coordinated care.
Candidate Profile
Essential Qualifications
- Current professional registration with the Nursing and Midwifery Council
- Master’s degree in Advanced Clinical Practice, or a relevant clinical Master’s degree with accredited ACP portfolio
- Training or experience in mentoring, supervision, or teaching learners
Essential Experience
- Significant experience in a Band 7 or equivalent clinical role
- Evidence of working autonomously in complex clinical environments
- Experience working collaboratively within multidisciplinary healthcare teams
Desirable Qualifications
- Teaching or education qualification
- Mentorship or coaching certification
- Clinical supervision training
