About the Opportunity

OneMedical Group is inviting applications for an experienced General Practitioner with a strong interest in palliative and end-of-life care to join the team at Minehead Medical Centre.

This role combines traditional GP responsibilities with specialist palliative care leadership within the community. You will help coordinate compassionate care for patients with advanced illness, supporting individuals and families through complex health journeys with dignity and respect.

Working closely with multidisciplinary teams—including community nurses, hospice specialists, social care providers and hospital colleagues—you will play an important role in improving the quality of end-of-life care within the local population.

Key Responsibilities

The successful candidate will:

  • Deliver high-quality primary care services within both the practice and community settings.
  • Provide specialist input in palliative and end-of-life care, supporting patients with complex or advanced conditions.
  • Lead advance care planning discussions, including DNACPR decisions and anticipatory care plans.
  • Optimise symptom control and medication management to improve patient comfort.
  • Work collaboratively with community, hospice and secondary care teams to ensure coordinated care.
  • Support colleagues within the practice through leadership, mentoring and service improvement initiatives.

Essential Requirements

Applicants should demonstrate:

  • Registration with the General Medical Council and inclusion on the GP Specialty Register.
  • Current listing on the NHS Performers List.
  • Post-CCT experience within general practice, urgent care or primary care services.
  • Evidence of training or experience in palliative medicine (e.g. diploma or relevant modules).
  • Proven ability to work effectively within multidisciplinary healthcare teams.
  • A proactive and organised approach to clinical and operational responsibilities.
