About the Role

We are looking for a dedicated and experienced Advanced Clinical Practitioner (ACP) to join our multidisciplinary Community Mental Health team. The post covers adults living in the Derwentside, Stanley, and Chester Le Street areas.

As an ACP, you will work autonomously across professional boundaries, providing assessment, diagnosis, treatment, and management for patients with complex mental health needs. You will also play a central role in service development, quality improvement, and leadership initiatives.

Key Responsibilities

Deliver high-quality, evidence-based care across the four pillars of advanced practice: clinical, leadership, education, and research.

  • Conduct comprehensive assessments and develop patient management plans.
  • Prescribe medications and implement interventions within your scope of practice.
  • Lead service improvement projects, mentor junior staff, and supervise clinical learners.
  • Collaborate closely with colleagues across primary, secondary, and community care settings.
  • Promote patient-centred care, ensuring patients and carers are actively involved in decision-making.

Person Specification

Qualifications & Registration

  • Current registration with the Nursing and Midwifery Council (NMC)
  • Master’s degree in Advanced Clinical Practice or a clinically relevant subject with e-portfolio accreditation
  • Preparation as a mentor, practice assessor, supervisor, or relevant teaching experience within the last two years
  • Substantial experience in a Band 7 or equivalent role demonstrating readiness for advanced practice

Desirable Qualifications

  • Teaching, mentoring, or coaching qualifications
  • Clinical supervision or leadership training

Skills & Knowledge

  • Ability to manage complex clinical care in partnership with patients, families, and carers
  • Experience working in multi-disciplinary teams and leading change initiatives
  • Strong communication, decision-making, and problem-solving skills
