About the Role
An excellent opportunity has opened for a Qualified Nursing Associate to join a well-established Community Mental Health Team serving the Derwentside and Chester-le-Street area.
In this role, you will support adults with mental health needs in a community setting, helping them work towards recovery, independence, and improved wellbeing. You will manage your own caseload as a Lead Professional, while being supported by a wider multidisciplinary team.
This position is ideal for someone who enjoys building relationships, working in the community, and making a meaningful difference in people’s lives.
Important Note on Eligibility
This role does not qualify for Skilled Worker visa sponsorship, so applicants must already have the right to work in the UK.
What You’ll Be Doing
Clinical Responsibilities
- Manage a caseload of service users as Lead Professional
Deliver recovery-focused interventions in the community
Support individuals in maintaining independence and improving mental health outcomes
Follow agreed care plans and escalate concerns appropriately
Teamwork & Collaboration
- Work as part of a multidisciplinary team (MDT)
Liaise with other services across the Trust and external agencies
Contribute to coordinated care planning and delivery
Professional Development
- Engage in clinical supervision and reflective practice
Support student learning and act as a role model for junior staff
Access ongoing training through Trust links with local universities

What You’ll Need
Essential Qualifications
- Registered with the Nursing and Midwifery Council as a Nursing Associate
Nursing Associate Foundation Degree
GCSEs (or equivalent) in Maths and English
Experience & Understanding
- Awareness of the Nursing Associate role within multidisciplinary care
Understanding of the Nursing and Midwifery Council standards of practice
Knowledge of when to seek support or escalate concerns
Familiarity with professional revalidation requirements
Skills & Attributes
- Strong communication skills (verbal and written)
Ability to manage and prioritise workload
Confidence working in dynamic and unpredictable environments
Ability to build positive relationships with:
Patients
Families and carers
Colleagues
Commitment to reflective practice and continuous improvement
Additional Requirements
Ability to travel across the community independently
Access to a suitable vehicle for work purposes (reasonable adjustments considered under the Equality Act 2010)
