Pacific Gardens Nursing & Rehab is a post-acute care facility offering long-term care, short-term rehabilitation, and geriatric outpatient services in Fresno, California. You’ll join a multi-discipline therapy team, receive ongoing training (e.g., Think Thin/dysphagia, dementia, stroke/neuro-rehab, respiratory recovery), and enjoy schedule flexibility. You’ll help our patients rebuild communication, cognitive and swallowing functions — and contribute to a team culture rooted in integrity, learning and collaboration.
What You’ll Do
- Screen, assess and evaluate patients with communication, cognitive or swallowing disorders; develop individualized care plans aligned with regulatory and clinical standards.
- Deliver rehabilitative treatment to patients with speech‐language, cognitive and/or dysphagia needs, and monitor outcomes; adjust treatment plans as needed and make referrals when indicated.
- Provide consultation and counselling to patients, families, caregivers and other interdisciplinary team members regarding communication and swallowing disorders.
- Generate thorough discharge summaries and treatment documentation in full compliance with clinical and regulatory requirements.
- Participate in continuing education and training programs (e.g., Think Thin, dementia/stroke/neuro rehabilitation, respiratory recovery programs) to maintain and enhance clinical expertise.
- Engage as a valued member of the therapy team, embodying our core values and contributing to a high-quality patient-care culture.
Schedule & Compensation
- PRN role: flexible scheduling offering strong work-life balance.
- Hourly pay: $70-$75 per hour, depending on experience.
Qualifications & Experience
- Valid Speech-Language Pathology (SLP) license required (California).
- Open to SLPs at all levels of experience — from early-career to seasoned clinician.
- Strong interpersonal communication skills and ability to engage patients, families and interdisciplinary teams.
- Interest in post-acute care, dysphagia/Think Thin training, neuro rehabilitation, geriatric therapy preferred.
