About the Role
We are seeking a dedicated Registered Nurse to join our Early Evening Service within NHS Forth Valley. This unique position bridges the day and night services, allowing you to work with local District Nursing teams in the afternoon before transitioning into the out-of-hours early evening service, which operates up to 8pm. Flexibility with shift patterns is essential to meet the needs of the service.
This role offers the opportunity to make a real difference in the community by promoting prevention, anticipatory care, and supported self-management, helping people to live longer, healthier lives at home or in a homely setting.
Key Responsibilities
- Provide high-quality nursing care across both day and early evening services.
- Work autonomously and collaboratively with District Nursing teams to assess, plan, and implement patient care.
- Deliver care in line with patient needs, including wound management, catheter care, and palliative/end-of-life support.
- Contribute to anticipatory care planning and proactive management of patients with complex needs.
- Support patients and families in managing health at home, promoting independence wherever possible.
- Participate in ongoing professional development and training provided during induction and throughout your career.
- Travel across the service area to deliver care efficiently and effectively.
Person Specification
Essential Skills and Attributes
- Registration as a nurse on Part One of the Professional Register.
- Excellent communication skills, both verbal and written.
- Ability to work autonomously and as part of a team, showing initiative and sound judgment.
- Willingness and ability to travel throughout the service area.
Desirable Skills and Experience
- Previous experience in community or district nursing.
- Clinical expertise in wound care, catheter management, and palliative/end-of-life care.
- Understanding of Anticipatory Care Planning and proactive patient management.
