About the Role:

In the Wallasey Adult Social Care Community Team, you will carry out strengths-based Care Act assessments and create person-centred support plans for a diverse range of service users aged 18 and above. The role aims to promote the wellbeing of adults with care and support needs, helping them achieve better outcomes and stronger community connections.

Requirements:

  • Registered with Social Work England
  • Relevant social work qualification
  • Evidence of Continuous Professional Development
  • Driving licence and access to a car for work purposes
