Diabetes Specialist Nurse – Acute Diabetes Team (Permanent Role)

About the Role

We are seeking an experienced Diabetes Specialist Nurse to join our Acute Diabetes Team in a permanent role. In this senior-level position, you will be responsible for assessing, developing, and delivering personalised care plans, while providing expert clinical advice in specialist diabetes management.

You will practice in accordance with your professional code of conduct and within your defined scope of practice—taking accountability for your decisions, actions, and omissions in line with professional expectations.

This post is aligned to the TREND Integrated Care and Competency Framework for Adult Diabetes Nursing at Senior Practitioner level.

You will collaborate with non-specialist clinical teams to develop and implement management plans for patients with complex or recurrent diabetes-related issues such as:

  • Persistent hyperglycaemia or hypoglycaemia
  • Hypoglycaemia fear or unawareness
  • Renal complications
  • Diabetic foot disease
  • Visual impairment
  • Preconception and pregnancy care planning

Your work will span both inpatient and outpatient settings across various specialty areas.

Key Responsibilities

  • Manage a defined caseload of patients requiring specialist diabetes care
  • Provide advanced clinical advice and support within the multi-disciplinary team (MDT)
  • Contribute to both strategic development and operational delivery of diabetes services
  • Practice independently while ensuring effective collaboration with wider healthcare teams
  • Deliver services within both community and acute hospital environments

Skills & Experience Required

Essential:

  • Registered Nurse with appropriate NMC registration
  • Postgraduate qualification or equivalent experience in diabetes care
  • Evidence of ongoing professional development
  • Strong leadership, communication, and teamworking skills
  • Ability to work autonomously and manage complex caseloads
