We are looking for a dedicated Community Staff Nurse to join our Community Nursing Team in the Aire/Wharfe area, based in Silsden. This is a rewarding opportunity to provide high-quality, person-centred care directly in patients’ homes—supporting individuals with a range of needs including long-term conditions, complex care, and palliative care.
What You’ll Do
As a key part of our Planned Care Service, you will:
Deliver holistic, patient-focused care within the community
Work a flexible, 7-day rotational shift pattern between 8am and 9pm
Use mobile technology to enhance the delivery of care
Collaborate with wider multidisciplinary teams and specialist services
Play a vital role in empowering patients to manage their own health
Development Opportunities

Requirements
NMC registration as a qualified nurse
Ability to travel extensively across the district
