Social Worker – Adult Social Care (Locality Team)

Salary: £40,755 – £48,531 per annum (up to £50,574 including unconsolidated maximum)
Annual Leave: 28 days (plus bank holidays)

About the Role

We are seeking experienced and dedicated Social Workers to join our Adult Social Care Locality Teams. You will play a key role in supporting adults to live independently and with dignity, tailoring support to meet individual needs.

You will work closely with colleagues across health and social care, taking part in a collaborative, person-centred approach to care planning and service delivery.

What You’ll Be Doing

As a Social Worker in our Localities team, you will:

  • Complete Care Act 2014 assessments and reviews to develop personalised support plans
  • Manage safeguarding enquiries, ensuring swift and effective responses
  • Conduct Mental Capacity Act assessments and make best interest decisions where necessary
  • Participate in the duty rota, as well as DSTS and CHC assessments
  • Undertake court-related work
  • Work collaboratively with families, carers, health professionals, and community partners to achieve the best possible outcomes

What We’re Looking For

We’re looking for compassionate and committed professionals with:

  • A Social Work degree (or equivalent) and current registration with Social Work England
  • A minimum of 2 years’ post-ASYE experience as a qualified social worker in the UK
  • Experience working with adults with complex physical and/or mental health needs
  • An understanding of multi-agency working and integrated care
  • Excellent communication skills and the ability to manage challenging and emotionally demanding situations
  • A focus on empowering individuals, supporting independence, and delivering positive outcomes
