Job Description
We are looking for a dedicated and empathetic Registered Mental Health Nurse (Band 5) to become part of our forward-thinking Community Mental Health Service at Somerset NHS Foundation Trust. Key Responsibilities
- Conduct detailed, recovery-focused mental health assessments that highlight individual strengths and needs.
- Provide tailored, evidence-based interventions to support recovery and positive change.
- Develop and sustain meaningful therapeutic relationships with service users across various mental health experiences, managing endings thoughtfully.
- Partner effectively with service users, families/carers, GPs, voluntary groups, inpatient teams, social services, and other agencies.
- Evaluate and respond to clinical risk, create safety plans, and engage in care coordination processes.
- Spot shifts in presentation or risk levels and raise concerns promptly through appropriate channels.
- Take part in supervision, team discussions, reflective sessions, and ongoing learning.
- Offer guidance and supervision to more junior colleagues (as arranged with the team manager).
- Keep precise, up-to-date records in line with Trust systems (such as RiO) and policies.
- Maintain the highest professional standards, following guidelines on safeguarding, confidentiality, information governance, clinical governance, and health & safety.
- Support service enhancements through audits, quality initiatives, and creative improvements.
Clinical & Professional Practice
As a Registered Mental Health Nurse, you will:
- Perform holistic assessments drawing on psychosocial, risk, and recovery models.
- Apply techniques like motivational interviewing, solution-focused methods, and strengths-based working to foster engagement.
- Co-create Recovery Plans and Risk Management strategies, sharing them with relevant partners.
- Handle sensitive and complex information with empathy and precision when communicating with service users, carers, and colleagues.
- Practice independently while benefiting from mentorship and supervision from experienced practitioners.
Collaboration & Partnerships
You’ll build effective links with a wide network, including voluntary sector providers, primary care, inpatient services, safeguarding teams, housing support, and substance misuse services. Where helpful, adopt a family/systemic lens in your work. You’ll contribute to multi-agency meetings, case discussions, discharge processes, and conferences.
Professional Standards & Development
Work in full compliance with the Nursing and Midwifery Council (NMC) Code (read the NMC Code here), alongside all Somerset NHS Foundation Trust policies, to ensure safe, effective care. Engage in quality audits, service reviews, and improvement projects. Keep clinical records accurate and respect data protection rules at all times.
Demonstrate leadership by role-modelling Trust values, supervising junior staff where needed, and helping with team induction, recruitment, and development activities.
Additional Requirements
- Flexibility to adapt to service demands and occasional changes.
- Willingness to travel across Somerset as the role requires.
- Completion of Prevention and Management of Violence and Aggression (PMVA) training when specified.
Person Specification – Essentials
Qualifications
- Registered Mental Health Nurse (RMN).
- Active registration with the Nursing and Midwifery Council (NMC).
Experience & Knowledge
- Solid understanding of health and social care resources and collaborative practices.
- Proven experience in assessing, planning, implementing, and reviewing care with service users and carers.
- Direct experience supporting people with mental health challenges (or equivalent personal insight).
Other Essentials
- Full UK driving licence.
- Subject to an enhanced Disclosure and Barring Service (DBS) check.
