About the Role
Pacific Gardens Nursing and Rehab, a respected post-acute care facility in Fresno, CA, is seeking a dedicated PRN Speech-Language Pathologist (SLP) to join our in-house therapy team.
Our facility provides short-term rehabilitation, long-term care, and geriatric outpatient services, all supported by a compassionate, interdisciplinary team. As an SLP, you’ll have the opportunity to work with patients in diverse clinical areas while gaining access to advanced training in programs such as:
- Think Thin Dysphagia
- Abilities Care and Dementia
- Stroke and Neuro Rehabilitation
- Respiratory Recovery

Key Responsibilities
- Screen, evaluate, and treat patients with communication, cognitive, and swallowing disorders, in accordance with professional and regulatory standards.
- Develop and implement individualized treatment plans to promote optimal functional outcomes.
- Monitor progress, modify interventions as appropriate, and ensure documentation meets clinical and compliance standards.
- Provide education and counseling to patients, families, and caregivers to support ongoing communication and swallowing management.
- Complete accurate discharge summaries and contribute to care team discussions and interdisciplinary planning.
Qualifications
- Current Speech-Language Pathology license (California required).
- Open to both new graduates and experienced SLPs.
- Strong interpersonal and communication skills.
- Commitment to evidence-based practice and patient-centered care.
