About the Role

Pacific Gardens Nursing and Rehab, a respected post-acute care facility in Fresno, CA, is seeking a dedicated PRN Speech-Language Pathologist (SLP) to join our in-house therapy team.

Our facility provides short-term rehabilitation, long-term care, and geriatric outpatient services, all supported by a compassionate, interdisciplinary team. As an SLP, you’ll have the opportunity to work with patients in diverse clinical areas while gaining access to advanced training in programs such as:

  • Think Thin Dysphagia
  • Abilities Care and Dementia
  • Stroke and Neuro Rehabilitation
  • Respiratory Recovery

Key Responsibilities

  • Screen, evaluate, and treat patients with communication, cognitive, and swallowing disorders, in accordance with professional and regulatory standards.
  • Develop and implement individualized treatment plans to promote optimal functional outcomes.
  • Monitor progress, modify interventions as appropriate, and ensure documentation meets clinical and compliance standards.
  • Provide education and counseling to patients, families, and caregivers to support ongoing communication and swallowing management.
  • Complete accurate discharge summaries and contribute to care team discussions and interdisciplinary planning.

Qualifications

  • Current Speech-Language Pathology license (California required).
  • Open to both new graduates and experienced SLPs.
  • Strong interpersonal and communication skills.
  • Commitment to evidence-based practice and patient-centered care.
