The Role:
As a Senior Practitioner, your primary focus will be to embed a strength-based approach to assessment and support, ensuring that people have greater choice and control over their well-being. Your role will involve applying the Prevent, Reduce, Delay principles of the Care Act to promote independence and enhance wellbeing at every interaction.
Key responsibilities include:
- Leadership & Supervision: Provide professional leadership and guidance to social workers and social care practitioners, ensuring high standards of care and improved outcomes for people.
- Monitoring & Reviews: Oversee regular monitoring and reviews of care and support plans to ensure the person’s needs are being met.
- Mentorship: Support and mentor social workers and assessors, helping them co-design innovative care plans with service users and carers.
- Case Management: Manage a caseload of complex cases, ensuring adherence to national and service guidelines while upholding the reputation of the directorate and council.
- Team Support: Assist the Team Manager in delivering high-quality services and support, working within agreed budgets and performance expectations.
