An excellent opportunity has arisen for an experienced and compassionate Senior Community Practitioner to join the MHSOP Scarborough Community Mental Health Team on a secondment or fixed-term basis.

This role is ideal for professionals seeking to expand their community mental health experience while contributing to high-quality, person-centred care. You will work within a supportive multidisciplinary team and play a key role in improving outcomes for individuals with complex mental health needs.

Key Responsibilities

  • Deliver comprehensive biopsychosocial assessments to evaluate patient needs and risks.
    Develop and implement evidence-based care plans to support recovery and independence.
    Provide preventative interventions to reduce hospital admissions.
    Work collaboratively with families, carers, and external partner organisations.
    Contribute to both pharmacological and non-pharmacological interventions.
    Offer clinical supervision, mentoring, and support to junior staff and care support workers.
    Participate in duty rota systems and multidisciplinary team (MDT) discussions.
    Ensure effective risk assessment, safeguarding, and care coordination practices.

Essential Requirements

  • Current professional registration (e.g., NMC, HCPC, or equivalent).
    Minimum 18 months post-qualification experience (including completed preceptorship/competency framework).
    Strong experience working with individuals with mental health conditions.
    Proven ability to work within a multidisciplinary team.

Knowledge of:

  • Mental Health Act and Mental Capacity Act
    Safeguarding procedures
    Clinical risk assessment and governance
    Evidence-based mental health interventions

Experience in:

  • Clinical supervision
    Mentorship or student support
    Quality improvement initiatives

Desirable Criteria

  • Postgraduate qualification in a relevant field
    Leadership or management training
    Non-medical prescribing qualification (for nurses)
