An exciting opportunity has opened for an experienced and compassionate Senior Community Practitioner to join a supportive and forward-thinking Integrated Community Mental Health Team.
Based at Princess Road Clinic, you’ll play a key role in delivering high-quality, person-centred care across the Whitby and Ryedale areas—supporting adults with a range of mental health needs in community settings.

 Your Role in the Team
As a Senior Community Practitioner, you will:
- Act as a care coordinator, managing a defined caseload
Deliver tailored care plans aligned with individual patient needs
Support patients through their full journey—from referral to discharge
Participate in the duty rota, responding to urgent needs
Provide guidance, supervision, and mentorship to junior staff
Support students and contribute to a positive learning environment
You’ll work within a diverse multidisciplinary team including nurses, social workers, psychologists, psychiatrists, pharmacists, and support staff.

 What You’ll Need
 Essential Requirements
- Current registration with a recognised professional body (e.g., NMC, HCPC)
At least 18 months post-qualification experience
Experience working with individuals with mental health needs
Ability to work independently and within a multidisciplinary team
Experience in supervision, mentoring, or student support
 Knowledge & Skills
- Strong understanding of risk assessment and clinical management
Knowledge of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
Ability to communicate complex information sensitively
Commitment to evidence-based practice and patient-centred care
Safeguarding and care standards
