About the Role
We are looking for a compassionate and experienced Senior Community Practitioner to join our supportive multidisciplinary team based at The Anchorage Clinic in Whitby.
This role offers a rewarding opportunity to work within the Whitby and Ryedale Integrated Community Team, delivering high-quality mental health care to adults in the local community. The clinic is ideally located near Whitby’s harbour, offering a calm and inspiring environment to work in.
You will play a key role in supporting individuals on their recovery journey while collaborating with a diverse team of professionals including nurses, psychologists, social workers, psychiatrists, pharmacists, and support staff.
Key Responsibilities
- Manage a caseload of service users as a named care coordinator
Deliver person-centred care plans tailored to individual needs
Support patients throughout their journey from referral to discharge
Participate in the duty rota system
Provide guidance and mentorship to junior staff and students
Contribute to a positive learning environment within the team
Work collaboratively within a multidisciplinary framework

Essential Requirements
Qualifications
- Current registration with a relevant UK professional body
Evidence of continuous professional development (CPD)
Minimum 18 months post-qualification experience
Completion of preceptorship or competency framework
Teaching/mentorship qualification (or willingness to achieve)
Level 2 proficiency in literacy, numeracy, and IT skills
Experience
- Working with individuals experiencing mental health conditions
Collaboration with patients, families, and carers
Experience in clinical supervision or mentoring
Working effectively within a multidisciplinary team
Knowledge
- Clinical risk assessment and management
Understanding of psychological care approaches
Awareness of UK legislation such as:
Mental Health Act 1983
Mental Capacity Act 2005
Skills
- Strong communication, especially in sensitive situations
Ability to take lead responsibility for care coordination
Teaching, supervising, and supporting others in clinical practice
Desirable Criteria
- Postgraduate qualification in a related field
Leadership or management training
Community-based mental health experience
Knowledge of quality improvement systems
Clinical skills such as venepuncture
