Role Overview
The Patient Pathway Coordinator (PPC) for the Diabetes Service is responsible for ensuring smooth and efficient patient journeys from referral to discharge. This role is ideal for individuals looking to gain experience in PPC administration or explore a new clinical specialty.
You will work closely with the Senior Patient Pathway Coordinator and Consultant Team, serving as a key point of contact for patients and clinicians.
Key Responsibilities
- Act as the main contact for patients accessing elective hospital services, providing timely updates and guidance throughout their care pathway
- Support the Consultant Team with administrative and coordination tasks
- Monitor and manage patient pathways, ensuring efficient scheduling and follow-up
- Maintain compliance with Trust Standard Operating Procedures (SOPs) and administrative policies
- Uphold high standards of patient care, demonstrating a customer-focused and professional approach
Person Specification
Knowledge & Skills:
- Proficient in computer use, with accurate keyboard skills
- Familiarity with NHS administrative systems and processes
- Understanding of databases and spreadsheets (e.g., PAS, Excel)
- Awareness of confidentiality, data protection, equality, diversity, and health & safety
Qualifications:
- GCSEs (or equivalent) in English and Maths at grade C or above
- RSA II (or equivalent)
- Desirable: ECDL, AMSPAR, IHRIM diploma, shorthand skills
Experience & Attributes:
- Experience delivering quality customer service
- Knowledge of NHS processes and patient administration
- Ability to work flexibly and as part of a team
- Strong organizational skills and attention to detail
- Desirable: Supervisory experience or managing health & safety
