About the Role
We are seeking a compassionate and motivated Health Care Assistant to join our Adult Community Nursing Service in Oldham. This role offers the opportunity to make a real difference by supporting registered nurses in delivering high-quality care to patients in their own homes and community settings.
As part of a multidisciplinary team, you will help ensure that patients receive safe, consistent, and personalised care while promoting physical, mental, and emotional wellbeing.
 Key Responsibilities
As a Health Care Assistant, your main duties will include:
- Delivering delegated clinical and therapeutic care to patients, families, and carers
- Supporting registered nurses with daily clinical tasks in patients’ homes or designated treatment rooms
- Observing, recording, and reporting changes in patients’ condition
- Working effectively as a member of the Adult Community Nursing Team
- Encouraging patient independence and wellbeing while promoting a safe environment
 Qualifications & Experience
Essential:
- NVQ Level 3 in Healthcare or Level 3 Senior Healthcare Support Worker Apprenticeship, or equivalent experience
- Level 1 Literacy and Numeracy (GCSE English & Maths D–G)
- Care Certificate (or willingness to complete)
- Previous experience in a health or social care setting, preferably as a care support worker
- Ability to work as part of a team
Desirable:
- Interest or experience in mentoring and supporting learners
- Commitment to Continuous Professional Development (CPD)
 Skills & Personal Attributes
Essential:
- Excellent written and verbal communication skills
- Basic IT skills for patient record management
- Strong organisational and time management skills
- Ability to travel across the Oldham community footprint to meet service needs
Desirable:
Full UK driving license with willingness to use own vehicle for work purposes
