About the Role

Minehead Medical Centre, part of the OneMedical Group, is seeking a compassionate and skilled General Practitioner (GP) with a special interest in palliative and end-of-life care. This role is perfect for GPs who are passionate about delivering high-quality, patient-centred care across the local community, including in patients’ homes.

As a GP with a palliative care focus, you will provide both generalist and specialist support to patients with advanced progressive illness and those approaching the end of life. You will also act as the clinical lead for palliative care in the practice, contributing to anticipatory care planning, symptom management, and the coordination of care across community teams, care homes, hospices, and secondary care providers.

Key Responsibilities

  • Deliver generalist and specialist palliative care across community and practice settings.
  • Lead anticipatory care planning, including advance care planning and DNACPR discussions.
  • Support symptom management, medication optimisation, and reduce avoidable hospital admissions.
  • Act as a clinical lead, guiding colleagues, contributing to service development, and ensuring high-quality standards.
  • Collaborate with community nursing teams, specialist palliative care teams, hospices, and social care providers.

Candidate Requirements

Applicants should:

  • Be a GMC-registered GP, on the GP Specialty Register and the NHS Performers List. See guidance on the General Medical Council (GMC)
  • Have post-CCT experience in primary or urgent care.
  • Hold evidence of postgraduate training in palliative care, e.g., Diploma in Palliative Medicine
    or RCGP End-of-Life modules.
  • Have a proven record of delivering excellent general practice care to patients with advanced illness.
  • Demonstrate experience in multidisciplinary collaboration across healthcare sectors.
  • Be organised, proactive, and passionate about patient care.
