About the Role

Minehead Medical Centre is seeking a motivated and compassionate GP with a special interest in palliative and end-of-life care. This role allows you to deliver both generalist primary care and specialist palliative input, ensuring patients with advanced progressive illnesses and those nearing the end of life receive coordinated, high-quality care across the local community and care homes.

You will act as a clinical lead for palliative care, guiding anticipatory care planning, symptom management, and seamless coordination with community teams, hospices, social care providers, and secondary care services.

This position offers flexibility, professional autonomy, and the opportunity to shape a critical service within a supportive, values-driven primary care at scale organisation.

Key Responsibilities

  • Deliver generalist and specialist palliative care across community and practice settings
  • Lead advance care planning and DNACPR discussions with patients and families
  • Support symptom management, medication optimisation, and avoidance of unnecessary hospital admissions
  • Act as a clinical lead, mentoring colleagues and contributing to service development and quality improvement initiatives
  • Collaborate effectively with multidisciplinary teams including community nurses, palliative care specialists, hospices, and social services

Candidate Profile

Essential Qualifications & Experience

  • Full GMC registration and on the GP Specialty Register
  • Listed on the NHS Performers List with post-CCT experience in primary or urgent care
  • Evidence of postgraduate training in palliative care, e.g., Diploma in Palliative Medicine, RCGP End-of-Life modules
  • Proven track record in delivering high-quality general practice, including care for patients with advanced illness
  • Experience in multidisciplinary collaboration across healthcare and social care sectors
