Joining the Hartlepool Community Interface Team, you will be part of a service designed around a “no wrong door” approach—ensuring that every individual receives timely support and is guided to the right care pathway.
Newly qualified nurses and final-year students (due to register within 3 months) are encouraged to apply. A structured preceptorship programme and ongoing training will help you grow in confidence and develop your clinical expertise.
Your Role in the Team
As a Community Mental Health Nurse, you will:
- Provide frontline mental health assessments and short-term interventions
Support individuals presenting with a wide range of mental health needs
Work collaboratively with patients, families, and carers to plan care
Signpost individuals to appropriate services and community resources
Manage complex cases in partnership with multi-agency teams
Contribute to continuous improvement and high-quality service delivery
This is a dynamic role where strong communication, sound clinical judgement, and compassion are essential.
Working Environment
You will be based at the One Life Centre in Hartlepool, working across the local community.
- Regular clinical and managerial supervision is provided
You’ll be supported by an experienced and approachable leadership team
Opportunities are available to visit the team informally before applying

What We’re Looking For
Essential Qualifications
- Current registration with the Nursing and Midwifery Council (NMC)
Registered Mental Health Nurse (RMN) or Learning Disability Nurse (RNLD)
Willingness to complete Practice Assessor/Mentor qualification
Evidence of ongoing professional development
Ability to undertake clinical supervision training within probation
Experience & Skills
- Experience working with individuals experiencing mental ill health or learning disabilities
Ability to deliver care within a person-centred, therapeutic framework
Experience of working within multi-disciplinary teams
Strong partnership working with families, carers, and external agencies
Effective communication and interpersonal skills
Knowledge Requirements
Understanding of:
- Mental Health Act
Mental Capacity Act
Knowledge of safeguarding principles and clinical risk management
Awareness of clinical governance and quality standards
Competence in medicines management, including administration
Important Information
- This role may be eligible for sponsorship under the Skilled Worker route
A Disclosure and Barring Service (DBS) check is required
